IMPORTANT:

  • We are a Cash ONLY practice offering Functional Medicine phone consultations.

  • We DO NOT participate in insurance or provide referrals.

  • All clients are required to have a Primary Care Provider in order to be seen.

  • Failure to Complete the Required Paperwork Will Result in the  Rescheduling of the Appointment. 

  • If new patients DO NOT give our office at least 72 hrs notification to reschedule their appointments, we will NOT reschedule any further appointments.
